BH is basically a darker version of QLAW, both about a childhood meeting between the prince and a noble girl, his determination to select her as his queen, and the palace scheming, intrigue, and assassinations that result in the FL losing her family and living a commoner's life before eventually ending up as the queen. The love/hate relationship between the main couple is similar, as is the FL's personality.

Both are darker, more intricate historical dramas with a similar tone and pacing. In both the FL's is originally the king's bride, but circumstances and political scheming result in her leaving the palace/being presumed dead. Eventually years later, she returns to the palace and returns to her position as queen. The ML in both is struggling against powerful people in the palace, all while trying to protect the woman he loves. Both stories have shamans.

the court politics are relentless. If you like the constant thrill of court politics in under the queen's umbrella then bloody heart would suit you as its similar but also carries a romance plotline alongside it.

Synopsis of Scarlet Heart:
When a total eclipse of the sun takes place, Go Ha Jin is transported back in time to the start of the Goryeo Dynasty of Korea during King Taejo's rule. She wakes up in the body of the 16-year-old Hae Soo and finds herself living in the house of the 8th Prince Wang Wook, who is married to Hae Soo's cousin. She soon befriends several of the princes and meets the ostracized 4th Prince, Wang So. Although knowing she should not get involved in palace intrigues over the succession to the throne, she inadvertently becomes a pawn in the struggle, as several of the Princes fall in love with her.

The environment and the ml are similar. Both are melodramatic and heavy on politics. Both deal with deep and dense characters. The cinematography is better in bloody heart but trolley is also good in terms of cinematography.
