This discussion just came up on the drama Hit the Spot, which has been labelled "extreme sexual content" and I disagree with that usage, the "sexual content" tag seems more than sufficent.
It seems to me that these tags are used inconsistently and the defintions aren't clear on what counts as what. What do we consider "extreme" and "very explicit". (outside of "oh this is obviously pornography" or should the extreme sexual content tag only be for erotica/porn?)
For example the movie Love Clinic has the same level of sex scenes that Hit the Spot does but only has the "sexual content" tag. The Handmaiden is another example which only has the "sexual content" tag. Should those tags be amended?
I think given that what's considered "extreme" sexual content may vary by person it may help to have clearer definitions of how we want these tags to be used on the site to help people in choosing what tags to add to a work. And to help other users in finding the kind of content they want to watch (or avoid).

Mild Sexual Content
The show depicts sexual behavior which is not very explicit.
Sexual Content
Material depicting sexual behavior which may be explicit.
Extreme Sexual Content
The depicting extreme sexual behavior which may be very explicit.
I suppose what I'm trying to understand is how are we defining "sexual behaviour" vs "extreme sexual behaviour" and "may be explicit" vs. "may be very explicit". Does simulated sex without clothing automatically put us into the extreme here? I imagine if I'm confused maybe others are as well?
